As of January 2008, the North Dakota Department of Health, which cares for all death records requires certified identification in order to request these public documents. This is due to the increase of theft as a result from ID fraud.

Here is the location of all forms you will need to complete your request: http://www.health.state.nd.us/vital/forms.htm

If you are interested in conducting genealogical research, try to verify which county the event took place in and contact the North Carolina State Archives. The North Carolina Vital Records is not able to offer records past a certain time period as this instead falls into the duties of the state archives.

North Carolina is also one of the only three states in the U.S.A. with a fraud protection program, utilizing a special type of paper which is hard to reproduce. Unfortunately, with identity theft hitting all new highs, taking steps like these are necessary to protect our information.

If you are seeking a certified copy or the records you desire, there is a $15 fee in addition to all other applicable fees.

death certificates can be requested:
By mail – All requests and fees can be processed by the:
North Carolina Vital Records
1903 Mail Service Center
Raleigh, NC 27699-1903

In Person – Polite staff are ready to assist you. There may be a line so come prepared as you may have a wait and always have the mandatory information ready when you are next.
North Carolina Vital Records
225 N. McDowell St.
Raleigh, NC 27603-1382

Electronically – Here is information about securing your documents on-line: http://vitalrecords.dhhs.state.nc.us/vr/rush/
VitalCheck is a handy resource to have if there is a long wait period for records. However, be advised there are extra fees in addition to the basic research costs which will end in costing you more than if you utilize North Carolina’s main office. If you encounter any problems, you may call 1-800-255-2414 or 1-800-669-8312.

Questions – You may call Vital Records: 1-800-669-8310.
