The death certificate may order in person, mail, fax for which the state defined fees for the death certificate $17 and $21 if processed through online along with application and other legal formalities.
To obtain the death certificate in the State of Colorado, the applicant must submit a proof of their relationship or establish their legal relation provided with the relaxation by the court order to obtain the death certificate of the citizen of the State. Birth certificate, marriage certificate or legal order may be the supporting documents, which helps the smooth process to obtain the death certificate.
In order to obtain the death certificate following documents are required by the State Registrar of Vital Statistics:
Copy of driving license, copy of identification issued by the State of Colorado, copy of school, college or university identification card, copy of US passport, Copy of foreign passport (Optional), copy of temporary/permanent resident card, certificate of US citizenship, tribal identification card, US military identification card, copy of U.S. Certificate of Naturalization (out of these whichever is applicable but no expired documents are acceptable).
